Novel interventions—including ECM remodeling, CAF modulation, metabolic reprogramming, and myeloid cell targeting—show promise in restoring immune responsiveness.ConclusionTME reprogramming represents a critical strategy to enhance immunotherapy efficacy in PDAC, offering new opportunities for overcoming immune exclusion and resistance.
